Participation & liability agreement Physical exercise and sports training can involve falls, collisions, equipment use, strenuous activity, and other risks of injury. I understand these risks and voluntarily choose to participate (or permit my minor child to participate). I understand that coaches do not provide medical diagnosis or treatment. I agree to follow posted facility rules and coach safety instructions and to disclose relevant participation restrictions directly to the program before training. I authorize staff to contact emergency services if reasonably necessary. To the extent permitted by applicable law, I agree to assume the inherent risks of participation and release JW Fitness & Personal Training LLC, Phoenix Sports Institute, Phoenix Sports Performance, Functional Integrated Training, Inferno Performance, and their respective staff and representatives from liability arising from ordinary negligence connected with participation. This does not waive claims that cannot legally be waived, including gross negligence or intentional misconduct. I understand that signing does not replace a physician's clearance where appropriate. I have read this agreement and sign voluntarily.
